|Description||Form from the State of Minnesota filled out by insurance companies to keep track of the number of policies written in the state. The inventory is two oversized pages of yellow cardstock printed in black. Printed and handwritten across the top of page 1 is "Minnesota/Statement of Fire, Lightning and Sprinkler Leakage/Direct Writings Within the Corporate Limits of the Municipalities/ Louisville Farmers Mutual Fire Ins. Co. Insurance Company of Scott Co. Minn/John P. Nachbar Sec'y/Jordan, Minn." The inventory was the business's 1955 home office copy. Each page of the form consists of four columns printed with the names of each city and township in the state. Each had their own alpha-numerical code. Next to the name of an area are lines for writing in the dollar amount for the company's direct writings received in 1955. The company filled in dollar amounts for Credit River Township, Glendale Township, Jordan, Prior Lake, St. Lawrence Township, Sand Creek Township and Spring Lake Township. Seen along the bottom edge of the last page is the signature and stamp of a Notary Public and signatures from the company's Vice President and Secretary.|
